Application Requirements:

Completed enrollment application.
Original Birth Certificate (a copy will be made and the original will be returned)*
Certificate of Baptism (catholic students only).
Copy of Social Security card.
State of Alabama Immunization Certificate.
Registration fee: $125 per student non-refundable.
Copy of most recent Grade Report (Grades 1 thru 8)
Interview with Mrs. Mary Tomaine, HFS Principal.
Custodial Care documents, where applicable.

*Students must be 4 on or before September 1 to enter the Pre-Kindergarten program and 5 on or before September 1 to enter Kindergarten.
